Additional Results Forms

If you have already taken the test with GVT Exams Inc. and are now applying to institutions, please follow these easy steps.

1 - Download and complete the two Request Additional Results forms (see on the right). Be sure to include any student numbers or file / case information at the bottom of the form.

2 - Fax / email the completed forms to GVT Exams Inc. You may also bring them in person. Be sure that payment is also included in the request. You can use the credit card form in the document, or use cash / debit in person. Time of delivery of results depends on where the institution is. Test Results Forms are despatched from GVT Exams Inc.
